South Eastern Occupational Therapy collects information about clients for the primary purpose of providing a quality service. In order to thoroughly assess and provide therapy, we need to collect some personal information from clients. If you do not provide the necessary information then we are unable to provide treatment.

Information collected may include personal details, medical details, observations and assessment results. In some instances photographs of clients, their environment or equipment may be taken in order to assist in the provision of occupational therapy services to clients.

Information collected may be used for:

  1. The administrative purpose of running the practice;
  2. Billing either directly or through an insurer or compensation agency;
  3. Use within the practice if discussing or passing your case to another practitioner within the practice for your ongoing management;
  4. Disclosure of information to your doctors, other health professionals or to teachers to facilitate communication and best possible care for you; and
  5. In the case of insurance or compensation claim it may be necessary to disclose and/or collect information that concerns your return to work to an insurer or your employer.

South Eastern Occupational Therapy has a Privacy Policy that is available to clients upon request.  The policy provides guidelines on the collection, use, disclosure and security of client information.
